Mandated Physicals

New York State Education Law requires that your student MUST have a physical exam report (Health Appraisal) on file in the Health Office if they are entering grades K, 1, 3, 5, 7, 9, & 11 and/or if they are a new entering student to the school district. This exam should be  done by your child's physician. An exam is acceptable if it has been completed up to 12 months prior to the beginning of the school year in which it is required.

 Please bring copy  of your child's current health appraisal to the school Health Office as soon as possible.  (Click here for link to Health Examination Form)

 Dental health certificates are requested, but not required, for students entering grades K, 1, 3, 5, 7, 9, & 11.
